you definently weren't pushing from your heels; at the top of the lift your heels are completely off the ground. Also in my opinion it would be better to post a video from the side; i don't think a front angle like that really shows anything.

Also that is the wrong way to take it out of the rack; walking backwards after squatting is never a good thing.

I'd think that you'd have a higher chance knocking it over when you are backwards rather than forward it might only be a matter of time. I'm having problems watching the video i will check back on it later.

I'm not going to critique the squat but will give you a few words of advice.....

Your squat setup looks very dangerous.. The rack looks very unstable and if I'm not mistaken.. you should have a couple of 45's loaded on the back of the rack to help stabileize it... ?

Backing up like that is not a smart idea... there is a bar going across the back about where the peak of your calf and it looks as if you're going to keep going and go straight through the plate glass window which could result in a serious injury, possibly decapitation...

Move into the rack front side first.. lift the bar off and take a small half step back.. the rack is equiped with safety rails that will probably hold up to 315lbs, so I think you'll be fine if you need to drop down..

I'm not trying to come down on you but this is a  prime example of how people get seriouly injured doing squats and various other exercises at home without proper setup or instructions..

and i agree.. put the camera to the side so others can have a better perspective of the lift.  Goodluck
There is never enough time to do everything, but there is always enough time to do the most important thing

Couple things, I noticed you come up on your toes after the first lift as well, didn't notice it on the other ones although I did notice you lift your toes off the ground and roll back on your heals which I imagine is really bad for your balance.  I know its nice to hit new numbers but there are some safety issues to worry about first thing, you want to be able to keep squatting the rest of your lift.  We definately could use a side shot for further critique, also your knees are going in and out on the way up, they really need to remain facing the way your toes are going to keep you from wearing out your knees.
