I think ephedra deaths can be put in the same class as this.

if you take too much creatine it turns into creatinine in the kidneys and it damages your kidneys and you just pee it out. just what i always thought

Documented cases? None that I know of. But does this mean there's no potential for long term damage? The studies I've seen have tracked the effects of creatine supplementation over a period of several years or (usually) less. If there are harmful non-immediate side effects from sustained creatine usage we probably won't know it for a while. This isn't to say that creatine is harmful, just that the lack of documented harm doesn't necessarilly make it safe. But I'm sure some of you will beg to differ...

All anecdotal: one case of high school wrestlers who donned thermal suits and sat in a sauna to make weight. Of course they died. It was not the creatine they were taking that killed them, but the creatine was blamed BECAUSE they were taking it.

We can set a more easily proven and scientifically valid standard (since proving a cause in a specific case is usually impossible)... Is there any evidence to suggest greater incidence of kidney disease or failure among creatine users compared to the population at large? (hint: the answer is the same)

Answer: Yes. There are a few documented cases. It involved those with pre-existing conditions of kidney (renal) issues known or unknown and one case of misuse of the product and taking too much.
